Recent reports of Shigella infections among school students in Kerala’s Wayanad have once again drawn attention to a disease that spreads quickly in communities, especially where children are in close contact. While most cases are mild and recover within a week, health experts warn that Shigella can trigger serious outbreaks if sanitation and hygiene measures are not maintained.
What exactly is Shigella?
Shigella is a group of bacteria that causes an intestinal infection known as shigellosis. The infection primarily affects the digestive tract and is best known for causing diarrhoea, stomach cramps, fever and, in some cases, bloody stools. According to the US Centers for Disease Control and Prevention (CDC), symptoms typically begin one to two days after a person is exposed to the bacteria.
How does it spread?
The infection is usually transmitted through the faecal-oral route, meaning bacteria from an infected person’s stool can enter another person’s mouth through contaminated hands, food, water, surfaces or objects. Poor hand hygiene after using the toilet, contaminated drinking water, improperly handled food and close contact in schools or childcare settings are among the most common causes of transmission.
Young children, particularly those under five years of age, are considered the most vulnerable group. Schools, hostels, childcare centres and other crowded settings can become hotspots if preventive measures are not followed.
Travellers to areas with poor sanitation and people with weakened immune systems may also face a higher risk of infection and complications.
What are the symptoms?
The illness often starts suddenly and may include:

- Stomach pain and cramps
- Nausea
- A frequent urge to pass stool, even when the bowels are empty
- Dehydration due to fluid loss
While many patients recover within five to seven days, some may continue to experience digestive problems for several weeks.
How is Shigella diagnosed?
Doctors generally diagnose shigellosis through a combination of symptoms, medical history and laboratory testing. A stool sample is usually examined to confirm the presence of Shigella bacteria and determine whether the strain is resistant to commonly used antibiotics. This helps doctors choose the most effective treatment.
Is there a cure?
The good news is that most people recover without specific treatment. Health agencies such as the CDC note that rest, adequate hydration and electrolyte replacement are often enough for mild cases.
However, severe infections may require antibiotics. Doctors caution that some strains of Shigella have developed resistance to multiple antibiotics, making laboratory testing increasingly important before treatment decisions are made. Anti-diarrhoeal medicines that slow bowel movement are generally not recommended because they can worsen the illness.
Why are outbreaks significant?
Shigella outbreaks are often viewed as indicators of gaps in sanitation, hygiene or food safety systems. In schools and community settings, one infected person can quickly spread the bacteria to others through shared facilities and contaminated surfaces.
Public health experts are also monitoring the rise of drug-resistant Shigella strains worldwide, which could make future outbreaks more difficult to treat. While severe complications remain uncommon, untreated infections can lead to dehydration, hospitalization and, in rare cases, life-threatening complications.
How can people protect themselves?
Health authorities recommend several simple but effective precautions:
- Wash hands thoroughly with soap and water after using the toilet and before eating.
- Ensure safe drinking water.
- Maintain proper food hygiene and cooking practices.
- Avoid sharing contaminated items.
- Keep children with diarrhoeal symptoms away from school until cleared by health professionals.
- Seek medical attention if diarrhoea becomes severe, bloody or prolonged.
